you can open console when u are playing and type:

/bind [key] needmedic Give me a heroine shot!

and when you press that key what u choosed, sound says: i need medic, but in chat you say: Give me a heroine shot!

or if you use config file for your binds, open it and type:

bind [key] "vsay needmedic Give me a heroine shot!"

i hope u understood what i mean :D

EDIT:
here is some help for making config for binds:

I understand what you mean Ninja but i got 1 problem... when i type /bind [ "vsay needmedic Give me a heroine shot!!" and press enter it will show the message but will not play the sound of "Need a medic!" and if i use other bind nothing will happen :? Maybe you can help me out with this 2:)

I think that is because it shouldn't be needmedic, it should be just medic like drag said (i was wrong :? )

so like this:
/bind [key] vsay medic Give me a heroine shot!
or like this:
bind [key] "vsay medic Give me a heroine shot!"
Image thx for diamond :D

You can also replace
bind [key] vsay medic Give me a heroine shot!
by
bind [key] vsay_team medic Give me a heroine shot!
In this way only your teammate will see this.
